BEFORE YOUR PROCEDURE

PLEASE NOTE: IF ALL OF THE BELOW ITEMS ARE NOT RECEIVED ON YOUR DATE OF SURGERY YOUR CASE WILL HAVE TO BE RESCHEDULED


PLEASE BRING* THE FOLLOWING WITH YOU ON YOUR DATE OF SURGERY:

1. LEGAL PICTURE ID
(Driver’s License or Passport)
Parents/Legal Guardians of minors are required to bring photo IDs.

2. HEALTH INSURANCE CARDS

3. CO-PAY / COINSURANCE / DEDUCTIBLE
(Our Insurance Verifier will call you prior to your date of surgery only if this applies to you)

4. DO NOT EAT OR DRINK after 12 midnight. This includes water, food, mints. You must be accompanied by a competent adult over 18 years of age with a valid driver’s license.

5. Please remove all jewelry and leave all valuable at home.

**** Our medical staff will also attempt to call the contact telephone number(s) you already provided to your surgeon’s office, prior to your date of surgery.
We will inform you of your arrival time and medical questions. Expect this “pre-op” phone call as your surgery date approaches. If you do not recive this call or if you require special needs, please call us at 352-536-6340

PRIOR TO YOUR SURGERY:

  • You will receive a link to our patient portal to fill out your medical history. This MUST be completely filled out for the clinical staff to effectively take care of your needs.
  • Follow all pre-op instructions given to you by your surgeon to prepare for you procedure.
  • DO NOT EAT OR DRINK after midnight leading up to your procedure day. This includes water, food, and mints.
  • Fill out the enclosed demographic form and bring it with you on the day of your surgery.
  • Our clinical staff will call you at least 24 hours prior to your surgery date with the time of your arrival. If you do not receive a call by 2:00 PM the day prior or if you require special needs, please call us at 352-536-6340.
  • Please make sure you make arrangements to have someone who can drive you to and from your procedure, if your procedure requires anesthesia. The driver must be over 18, have a valid driver’s license, and be able to stay on the premises the entire length of your stay at the Center.
